Do Breast Implants Cause Cancer?

Patients frequently show up at their breast augmentation consultation appointments consultation appointments and end up asking the question, “Do breast implants cause breast cancer”? 

Unfortunately, the answer is quite complicated. 

In short, it’s possible, but rare. The longer answer is not nearly as simple. So, let’s dive in. 

Do Breast Implants Cause Cancer?

Although breast implants have been around since 1961, it wasn’t until 1997 that a woman was found to have developed a rare lymphoma called anaplastic large cell lymphoma (ALCL) related to her breast implants. Today, this lymphoma is called breast implant associated anaplastic large cell lymphoma (BIA-ALCL).

Reports in recent media of breast implants causing breast cancer are referring to BIA-ALCL, which is not related to primary breast cancer. It’s very important that patients understand the difference between BIA-ALCL and breast cancer as they are two totally different diseases with very different prognoses and treatment options.

How BIA-ALCL Differs From Breast Cancer

Do Breast Implants Cause Breast CancerThere are several factors which make these two types of cancers very different. The most important factor is that because BIA-ALCL is a lymphoma, it is a disease originating from the immune cells in the body and not from the breast tissue.

To date, there have been a little over 300 reported cases of BIA-ALCL worldwide. That’s in over 50,000,000 women with breast implants. 

Furthermore, these women all have one thing in common: They all had breast augmentations with textured silicone breast implants. As far as we know, there has never been a case of BIA-ALCL in a woman who had smooth breast implants.

Do Silicone Breast Implants Cause Cancer?

It’s not silicone implants that are likely the causal factor with BIA-ALCL, but rather, the textured surface of some implants. Many patients get these features confused, so when they ask, do silicone breast implants cause breast cancer, it’s important to clarify the difference between silicone smooth implants and silicone textured implants. 

Generally speaking, breast implant surgery with textured implants is performed when a patient wants teardrop or anatomical implants. The teardrop shape requires texturing so that the implants don’t rotate inside the implant pocket and cause an unnatural appearance.

So, what is it about textured breast implants that makes BIA-ALCL only occur in some patients? We believe the texturing on the breast implant allows for bacteria to hideout within the nooks and crannies of the silicone shell. Bacteria are smart, and they know that as soon as they enter the body, the immune system will be looking for them. 

Therefore, they do something very unique: A few bacteria cells make a covering over themselves called a biofilm. This biofilm acts like a barrier between the bacteria and the immune system.

So, even though the body knows the bacteria is there, despite its best efforts, it can’t get to it to destroy it. What happens over time is that the normal immune cells get frustrated trying day and night to get to these bacteria, and then the immune cells start to mutate. This mutation then prevents the cells from acting as they normally do.

Over a long period of time, these textured breast implants raise the likelihood that the formation of a lymphoma may occur. Most women presenting with BIA-ACLC get through their recovery time for breast implants just fine and don’t have a problem for many years. It’s usually those who’ve had their breast implants for 10 to 15 years who end up showing symptoms after this time. This is because the cellular transformation process takes a very long time.

Symptoms of BIA-ALCL

Here are some of the symptoms that may be related to BIA-ALCL:

  • Breast swelling
  • Breast hardening
  • Lumps in the armpit or breast
  • Breast pain
  • Red skin rash on or around the breast

Cosmetic Breast Augmentation and Cancer: Is BIA-ALCL Treatable With Breast Cancer Surgery?

Do Silicone Breast Implants Cause CancerThe promising news about breast implant associated anaplastic large cell lymphoma is that it is very treatable. Current studies recommend removing the breast implant or implants as well as the scar tissue capsule around the implants (note: breast implants before and after the initial healing period post-augmentation look very different; the formation of scar tissue around implants occurs over time and is completely normal). Scar tissue and implant removal is curative of this rare cancer in the vast majority of cases.

Cancer Risk and Death Rate

So, what is your actual risk of dying from BIA-ALCL if you have textured breast implants for your entire life?

In order to understand this question we need to introduce a term called a micromort. One micromort is a one in 1,000,000 chance of dying from a certain activity. 

The concept of micromorts was originally developed to help evaluate high risk activities such as mountaineering or skydiving, as well as other common everyday activities. For instance, drinking 0.5L of wine, living two days in NYC, riding a bicycle for 17 miles, riding in a car for 230 miles, or traveling by plane for 1000 miles all increase your risk of dying by one micromort. 

In terms of BIA-ALCL, the micromort risk for a woman with bilateral breast implants over her lifetime is only 0.4 micromorts. That means you’re five times more likely to die every time you drink a bottle of wine or spend four days in New York City, than you are from BIA-ALCL related to textured implants!

Is the Risk Worth It?

The short answer is although there are reports of breast implants causing cancer, the incidence of BIA-ALCL is extremely low and if a woman is diagnosed early, the disease is very treatable.

Although some of your mini BBL results should be obvious directly after surgery, it will take up to six months for the full results from your surgery to be visible. During this time, it’s important to follow your plastic surgeon’s recovery instructions.

Mini BBL vs BBL Results

Mini BBL Vs BBLIn order to understand the recovery period and when patients will see their results after a mini BBL, it’s first important to recognize the differences between a mini BBL and a typical BBL procedure. 

Of course, both types of surgery involve the same basic procedure: Fat cells are suctioned out of the belly, lower back, thighs, or upper arms. Next, the fat cells are processed in a way that allows them to be reused. And finally, the fat cells are re-injected into the hips and buttocks to add volume and contour.

What makes a mini BBL slightly different than a regular BBL is that a mini BBL is targeted at patients who are overall slim and athletic. Naturally, the key issue here is that these patients won’t have a lot of excess fat to work with — but they’re still looking for some body contouring.

While some plastic surgeons won’t perform a BBL on such patients, Dr. David Sieber actually developed the mini BBL (skinny mini BBL) because he saw a direct need for this type of procedure. 

Dr. Sieber’s office also tends to see slim, athletic patients frequently. These patients are just as interested in reforming or slightly altering the shape of their buttocks, hip, and belly area, so the mini BBL is perfect for them.

How Mini BBLs Are Different

The mini BBL alternative approach that Dr. Sieber developed is specifically designed for patients who don’t have a lot of spare fat. Dr. Sieber carefully assesses the anatomy of these patients to find areas where fat can be safely removed while maintaining the patient’s aesthetic goals. 

Generally speaking, fat is removed from one or more of the following areas: the abdomen, back, flanks, inner thighs, or upper arms. Once the fat is harvested, it is processed as with a traditional BBL, then re-injected into the hip and butt area. 

Frequently, this type of fat transfer procedure will be used to create more of an hourglass figure for women. It can also fix or fill in “hip dips” and reduce “muffin tops”.

Mini BBL vs Traditional BBL Results

Mini BBL Near MeWhen it comes to your results from mini BBL surgery, the key difference is in the extent of the results. Mini BBL results tend to be less dramatic. The timeline of recovery, however, is basically the same. 

For the first few days, you may have some discomfort and pain. Pain medications can help reduce your discomfort. You should notice some results right away, but you’ll also have a lot of swelling, which may skew what you see when you look in the mirror. Be patient, and give it time.

For the first week or two, it’s important to rest and avoid putting any pressure on your hips or buttocks. Special pillows and chairs can help keep you comfortable.

Understanding Liposuction Before and After

Numerous individuals face the challenge of excess fat on their bodies. Despite efforts with exercise and a healthy diet, these stubborn fat bulges may persist. In such cases, liposuction, often referred to as “lipo,” becomes an excellent choice to attain the desired body contour.

This procedure effectively targets and removes excess fat deposits from specific areas, leading to a slimmer and more refined body shape, ultimately enhancing proportions and contours.

An important concept to understand thought is what liposuction doesn’t do. For instance, liposuction doesn’t tighten loose muscles or skin. If your tummy jiggles, it will likely jiggle after your procedure. If your muscles are loose, they’ll be loose after lipo. If you have stretch marks or cellulite on your thighs or stomach, you’ll have them after your procedure.

While liposuction may help you to lose some weight (generally one to ten pounds), that’s not the primary goal of liposuction. Lipo is intended to help you better shape and contour your body in the areas you’re concerned about. You should also know that like with any surgery, there are risks, which you should discuss with your plastic surgeon.

ARE YOU A GOOD CANDIDATE FOR LIPOSUCTION?

GETTING PREPARED FOR LIPO

Your first step will be to consult with the plastic surgeon. Discuss your goals, the benefits and risks, the options and costs. Make sure you ask them all your questions. If you do decide to have lipo, your plastic surgeon will provide you with instructions on how you should prepare for it properly.

These may include:

  • Telling you about alcohol and diet restrictions.
  • Asking you for information on any allergies you have or if you’re taking any medications, including herbal and over-the-counter supplements or drugs. They’ll likely ask you to stop taking specific medications like certain painkillers or blood thinners a few weeks before your procedure.
  • Telling you to have someone to take you home after your procedure.
  • Being prepared that you may need to stay overnight if you’re having a great deal of fat removed and you need to have the surgery performed in the hospital.

Before your procedure begins, the surgeon may mark up the body areas you’ll receive treatment. They might also take photos to use for “before and after” pictures.

Your surgeon may give you general anesthesia, meaning you will be asleep during the procedure. In some cases, you might only need local anesthesia, meaning you’ll be awake, but won’t feel any pain.

WHAT TO EXPECT AFTER LIPOSUCTION

After your surgery, you can expect to experience some:

  • Mild discomfort
  • Redness
  • Bruising
  • Swelling

Your doctor may prescribe you medication to ease this discomfort. You’ll likely need to wear a compression garment to promote healing and reduce swelling. Swelling typically starts reducing after your first week, but you’ll still need to wear the compression garments until week four or as your surgeon directs. You may experience swelling that lasts longer than six weeks, however, usually within a few days, you’re comfortable enough to go back to work.

During the procedure, the surgeon will make small incisions to your abdomen to remove the extra fat cells. They’re made in inconspicuous places where they’re less noticeable. The surgeon will provide you with post-care instructions for these incisions after your surgery. The instructions will help ensure they heal with minimal scarring.

It’s likely you won’t see immediate results. If anything, you may even feel like you’ve gained weight or appear larger in the treated area for a short time. This is normal. Although lipo can eliminate up to 10 pounds of fat cells, swelling and fluid retention can make you appear bloated. Once the swelling begins going down, you’ll start seeing results.

You might also notice some of your skin in the treated area appears loose. This skin should firm up over time. But, depending on various factors like how much fat was removed and your age, you might require surgical intervention to tighten up the skin. Each individual patient is different and your surgeon will talk about skin tightening with you as part of your body contour procedure if needed.

Depending on the amount of fat removed, you should expect to see your final results within one to three months following your liposuction procedure. With just one procedure, you can expect noticeable changes and improvements to your body’s contours. Remember, results may vary for each individual, and before and after photos and content do not guarantee specific results.
